I think in a video by vepr12allday I saw where the site ladder had been removed and a rail installed with a small red dot installed? I can't find anything on it, maybe I'm not searching with the correct words? Can someone point me in the right direction? The one in particular I'm talking about looks to be flush with the rear rail and rail or top if gas tube. Not the one that sticks up high.

the gas tube rail?

You can replace the gas tube with a gas tube that has a rail built into it.

or the chaos rail which gives another 922r part replacing the russian handguard

That will require a piece of steel piccy rail, some grinding and then need to be tigged on. There are after market options that replace the rear sight but they stick up higher. If you want it flush then its really a gunsmith or engineers job.

I just watched a vepr12allday video on the scout mount and he mentioned that what I was after is as you said, done by welding the piece in.

I wonder if it could be done using the existing points where the sight is able to swivel and the dust cover swivel, a pic rail with threaded tubes welded to the end and screws used in front like the scout mount and something that can easily be taken out for the dust cover swivel point?

Someone makes that and it will sell. :D

Dissident Arms Ultra Low Sight Rail.

Sight base is milled level to accept a 1913 picatinny rail, which is then fuse welded in place by a master structural welder.

Lowest possible sight base. Most solid possible sight base.

You simply cannot do any better.
